In December 2021, Surgeon General Vivek Murthy issued a rare warning: Mental health challenges were leading to “devastating effects” among young people. His statement came as the suicide rate for young Americans ages 10 to 19 jumped by 40 percent from 2001 to 2019, while emergency room visits for self-harm rose by 88 percent.
Lately, Dr. Murthy has been using his position to highlight the issue, much as Dr. C. Everett Koop, who was surgeon general in the 1980s, famously addressed the dangers associated with smoking. Dr. Murthy has been on a listening tour of sorts, speaking with students, health care workers and community groups across the country
He recently spoke with The New York Times about the growing intensity of his focus on this issue. This conversation has been edited for clarity and brevity.
You have described mental health as “the defining public health crisis of our time.” Why?
I think of mental health as the fuel that allows us to show up for our communities, our friends, our family and our lives. And when that fuel tank is low, it compromises our ability to be who we can be in our best moments and to show up at work or in school.
The social media platform Instagram made headlines last year for suppressing likes in an effort to curb the comparisons and hurt feelings associated with attaching popularity to sharing content. But do these efforts combat mental health issues, or are they simply applying a band-aid to a wound?
It’s a small step in the right direction, says Jacqueline Sperling, PhD, a psychologist at McLean Hospital who works with youth who experience anxiety disorders, about Instagram’s recent restriction.
“Even if you remove the likes, there continue to be opportunities for comparisons and feedback. People still can compare themselves to others, and people still can post comments.”
Some experts worry that teens are more anxious and have lower self-esteem because of social media and texting. There are key differences to socializing online. Teens miss out on things like body language and facial expressions. This can lead to misunderstandings and hurt feelings. It can also make talking in person feel more intimidating.
It is also common for kids to feel bad about themselves when they see everyone online looking perfect. Teens often try to compensate by sharing pictures that make them look perfect, too. Then, when their social media identity doesn’t match how they actually feel, they can end up feeling worse.
Parents can help by setting a good example of how to use tech. Try to give kids your full attention when you are with them, and establish tech-free zones in in the house and tech-free hours when no one is on their phones.
To help build self-esteem, get kids involved in something they’re interested in. When kids learn to feel good about what they can doinstead of how they look and what they own, they’re happier.
